Submitting the form creates a website-server enquiry record and reference. It does not automatically create a hospital appointment, clinical contract or travel booking. Relevant providers must confirm services and appointments.

Service information
Relevant providers explain and confirm consultation formats, fees, medical services and travel arrangements in writing. General information does not replace clinical advice or individual provider instructions. Official information governs visa and entry matters.
Account use
Use an email you are entitled to use, manage your account responsibly and do not submit another person’s sensitive information. You can update details, reset a password and delete an account. Development emails go only to the local mock outbox.
